Physicals, Concussion & Covid Forms:

All athletes are required by the state to have a current physical on file at the school. Sports physicals are only good for one year.

All athletes, parents, and coaches are required by the state to fill out a concussion form and a Covid form. Those are also only good for one year. All of the forms can be found here on the website.
